The information obtained in this certification process will be used by Greeley Evans Transit only for the provision of transportation services. Information will only be shared with other transit providers to facilitate travel in those areas. The information will not be provided to any other person or agency.
Before you begin the application, download the Request for Professional Verification Packet. Print off the verification packet to take to your Physician/Health Care Provider to complete and send back to Greeley Evans Transit.
In addition to the form below, we will need you to complete an authorization form for us to contact your physician or health care professional to confirm your information.

Once Greeley Evans Transit approves paratransit applications, rides can be booked one to 14 days in advance by calling 970-350-9290 during normal business hours or by filling out the ride booking form. A driver in a zTrip vehicle will complete the booked ride.
